The National RN Case Manager Training Center is committed to the education needs of Registered Nurses to prepare for new and emerging case management and care coordination roles within their practice settings. Our courses deliver high quality, evidence based content designed to support RNs in their ability to acquire new knowledge quickly and efficiently.
Content focuses on application in practice and addresses quality measures, patient-centered multidisciplinary teams and improved patient engagement and satisfaction. Courses are offered in both online and face-to- face + online (blended) learning formats. Use of our eLearning platform/classroom allows content to be delivered using a variety of learning methods that include faculty lecture, videos, recommended readings, as well as, evidence based clinical tools and resources. Courses are fully vetted and curricula are developed and taught by expert faculty. Case Management/Care Coordination Program Description of program Case management strives to assist patients and families to navigate through healthcare system. Case managers provide advocacy for patients and interact with the members of the healthcare team to find solutions in providing effective, quality, and efficient care. Case managers provide care coordination to communicate the needs of the patients and guide the delivery of appropriate and effective care. The program was designed utilizing the Standards of Practice for Case Management as endorsed by the Case Management Society of America. This program is designed for English-speaking nurses, social workers, and other healthcare professionals who want to enhance or develop the knowledge and skills necessary for quality case management. Program Content The content is divided into modules. Each module utilizes instructor outlines, audio/video recordings, reading assignments, articles, web resources, case studies to augment the learning process. These methods provide the participant with flexibility in completing assignments and interacting with the faculty. Each module has accompanying study guide(s) with questions (such as multiple choice, essay) for participants to submit to faculty. Interaction with faculty may occur as feedback to study guides, discussion boards, or email. The modules cover case management principles, case management process, care coordination of services, financial aspects of case management, disease management, and application of care management.

Course Schedule This eight-week activity is offered three times a year. The deadline for registration is the Monday prior to the start date by 8 a.m. 2018 Dates Start End March 26 May 18 June 4 July 27 Aug. 5 Learning Outcome The learning outcome of this program is to enable the participant to successfully implement care management (care coordination) practices, ethics, and standards in their clinical settings. Program objectives At the conclusion of this program, the participant will be able to:. Present an overview of the case management process. Identify methods to coordinate services to meet client needs.
Explain the financial aspects of case management. Integrate disease management and case management into the plan of care. Apply case management principles to clinical situations. Delivery of content The content is delivered asynchronous via the online learning management system, Blackboard. The internet provides an opportunity for interaction with program faculty while in the comfort of learning in your own home and office.
Program staff are available during business hours by phone or email to assist participants with technical issues or concerns. Note: Many businesses, especially government agencies or health institutions, utilize security firewalls. Access to web links within program and communication from the instructor, or other students may be blocked. We recommend not using your work email address for this course. Contact hours 40 contact hours Criteria for successful completion. Submit registration and payment. A certificate for contact hours will be awarded following completion of each module study guides, receive a passing score of 70% or better on comprehensive final, and submission of program evaluation.
The certificates will be sent electronically within approximately 30 days following the completion of the program. Successful completion of this program does not guarantee expertise in the content. Clinical experience and judgment coupled with an understanding of the content are necessary to attain competency. Achieving certification via a designated professional organization is an additional step to demonstrate competency.
